Job Description Sentara Home Care in Williamsburg is seeking an experience Home Health - PRN LPN
Job Responsibilites:
Assist in assessing, planning, implementing, and evaluating under the direction of a RN nursing care of patients in their place of residence. Carries out nursing skills contributing to treatment and rehabilitation. Plans for coordinated nursing service for individuals and families under their care. Assessment of individual and family health needs and hazards, existing or potential. Provides health counseling, including emotional/spiritual support. Consults with and refers families to appropriate community services. Documents all observations and procedures and performed. Supervised by the registered nurse at least every 30 days.
Minimum Requirements:
- Graduation from an accredited school of practical nursing. -1-2 years experience in medical surgical nursing required
- Experience in home health or public healthstrongly preferred.
- Must be able to recognize the state of wellness order to relate the underlying illness and to take appropriate action.
- Must be licensed by the Virginia State Board of Nursing - LPN. Valid drivers license. - Constant contact with patients in varying degrees of stress; patients family, and other professional personnel in the performance of duties. Must maintain the confidentially of the patients chart and condition. Must accurately report on patient's condition and any other pertinent information occurring in the home. Error could result in harm to the patient. Lifts, positions and moves patients in various weights.
